Hi,Can I apply this over oil-based spar URETHANE and Varathane wood stain to block the VOC for an exterior door? Thanks.

No. EXT is a water-based finish, so it will not bond to anything oil-based. Water and oil do not mix.If you are concerned about VOCs, your best course of option would be to sand the door down to the raw wood and then re-stain with a zero VOC stain, such as Rubio Monocoat Hybrid Exterior Wood Protector.
