How often does this need to be reapplied to limestone floor tiles? (some in a bathroom/laundry room with no shoes, and some in an entry area with shoes)

Reapplication may be required every few years depending upon the type and amount of foot traffic, the type and frequency of cleaners used as well as other environmental conditions. It also depends upon how many coats you apply and how porous is the the limestone. Sorry, it's hard to give a simple answer due to all the variables.
