I want to paint an old fireplace hearth. Is this product opaque enough to fully block old deep set stains or do I need to first use a primer?

Always prime first to block the stain from seeping through the paint. AFM Transitional Primer works best on oil or water-based stains. Use one coat and allow to dry 24 hours. Then apply two coats of the Concrete Floor Paint.
